Direct Support Providers are needed for a professional with spinal muscular atrophy type 2. I am in need of one or two assistants to work in my home and the community. I need help with things like transfers, dressing, showering, using the toilet, housekeeping, meal prep, etc. I am an active wheelchair user who works from home the majority of the time. In the community, I need an assistant to drive me and provide assistance with personal care. I have my own accessible vehicle, but I need someone to drive.

I love trying out new recipes, listening to all kinds of music, watching movies, crafting, snuggling my dogs, and doing things outside. I am looking for someone with a good work ethic, a positive attitude, and a good sense of humor. I am a positive and patient person. I do not expect perfection and will work with someone who is really trying.

Duties/Assist with:
Showering / Toileting / Dressing
Housekeeping/Organizing
Taking Medication/Using equipment
Range of Motion/Exercise
Skincare
Preparing meals
Care/Cleaning of equipment
Running errands/Transporting me in my van
*There is no direct lifting as I use a Hoyer Lift for all transfers. I also have a shower chair and a roll-in shower.

Requirements:
Must be over 18 years old
Must be able to pass a national background check and drug test
Must be able to legally work in the U.S.
Must have reliable transportation
Must have a valid driver's license with a clean driving record

Preferred:
CPR and First Aid certifications
Experience in this field
No smokers

*No heavy lifting, but the position requires bending, squatting, and pulling.

Shifts: Sunday-Saturday, 50 hours each week. Time and duration may change, but with notice. The Assistant must call and let Samantha know they will be late. Assistants must give as much advance notice as possible if they are unable to come to work. If the Assistant is ill or running a fever, they should let Samantha know and not come to work until they are not contagious. Masks and gloves will be provided if the Assistant does not have them.

Salary: $12.65 per hour during orientation and training. Pay is bi-weekly and negotiable after training is complete. Small bonuses at least twice annually will be provided. The Assistant will be provided with a Pay Schedule as well as a Shift Schedule upon hire.
