Wife/mother currently suffering from apparent senior health issues. Needs continuous companionship. Has tendency to rise from sleep suddenly.
No violent behavioral issues, but does not follow simple instructions, or remain seated or lying in bed.
Usually understands instructions. Needs help going to and using bathroom.
Recommend initial visitation to observe patient and make further recommendations.
