2. How long have you known the applicant? Mrs. Monohan has known Christopher since May 2005.
3. What was the nature of your relationship to the applicant? (For personal references) Mrs. Monohan is the supervising parent who along with her husband hired Chris Gibbons as a nanny for the family's 2 children ages, 2 and 5. The five year old is a special needs child.
4. What were the responsibilities of the applicant? Chris is responsible for getting the children up in the morning and getting the 5 year old ready for school. He needs assistance with every aspect of daily living. Chris dresses him, brushes his teeth, helps with toileting, and feeds him and keeps his feeding tube clean. He sees that he gets on the school bus safely and takes care of him after school. The rest of the day he cares for the 2 year old. He prepares meals and snacks, dresses the baby, changes her, plays with her, and fully engages her in age appropriate activities. He also reads to her, and watches out for her safety as well as works with her developmental and educational needs. Although Chris is not required to clean the house, he is responsible for keeping the children's things clean and in order.
5. What were the applicant's strengths? Chris is patient, reliable, responsible, and trustworthy and has high ethics. He is also loving, caring and has a sweet demeanor. Chris also knows sign language which is necessary since the 5 year old is non verbal. Mrs. Monohan said that she never has to worry about leaving her children and feels very comfortable with Christopher's care of them.
6. What kind of attitude did the applicant usually have? Chris has a very good attitude.
7. What, if any, challenges did you face in dealing with the applicant? The only area that Chris needs to be reminded of is cleanliness. Mrs. Monohan said that he is great with the children and just needs to know what is expected of him. He is very willing to comply.
8. Why did the applicant leave? The family just moved farther away, and since Chris needs to be at work at 4am he has decided that it is too far.
9. Would you hire the applicant again? Yes. For sure.
10. Would you have any hesitation or reservatiions about recommending the applicant to serve as our nanny or caregiver? No.
11. Are you aware of any physical, mental or emotional disabilities that would possibly permit this candidate from fully performing their duties as a childcare provider? No
12. Room for improvement? The area of cleanliness was the only thing she mentioned and felt Chris could be a bit more self initiating in this.

4. What were the responsibilities of the applicant? Chris was in charge of the morning prep and cashier duties. These jobs required but were not limited to arriving on time, accurately reporting any mistakes in transactions, taking and relaying orders down the line, dealing with irate customers and reporting daily sales.
5. What were the applicant's strengths? Chris was one of the most reliable workers that I've supervised. He always showed up on time and stayed late on many occasions to help take the load off fellow employees. He was honest and consistently dependable.
6. What kind of attitude did the applicant usually have? Chris was a mild mannered and cool tempered individual. He was upbeat and pleasant to work with.
7. What, if any, challenges did you face in dealing with the applicant? Starbuzz Coffee can be a very hectic environment. Customers are often in a hurry and Chris sometimes would get flustered under pressure.
8. Why did the applicant leave? Chris relocated and transfered to another store.
9. Would you hire the applicant again? Yes, he was one of my best employees yet.
10. Would you have any hesitation or reservatiions about recommending the applicant to serve as our nanny or caregiver? Although I didn't supervise Chris in an environment where he interacted with children often, he is a responsible individual and I don't see any reason why he wouldn't be qualified.

2. How long have you known the applicant? Mr. Drake has known Chris since June 2000.
3. What was the nature of your relationship to the applicant? (For personal references) Chris was in Mr. Drake's improv group for many years. After Chris moved, Mr. Drake and Chris continue to keep in touch.
4. What were the responsibilities of the applicant? Chris was an integral part of the group. Outside of his on stage duties, Chris was also in charge of equipment check in. Before and after each show, Chris required all of the other members to sign equipment in and out.
5. What were the applicant's strengths? Chris pays an amazing attention to details. He maintained a log of everything and where it was located. Often during the hectic times before a show people would ask Chris where items were located because he was always sure to know where something was located.
6. What kind of attitude did the applicant usually have? Chris was beaming with happiness. Even when he received less than positive audience reviews, he looked at it as an opportunity to improve.
7. What, if any, challenges did you face in dealing with the applicant? Chris sometimes had issues with his punctuality. After being confronted for being late 3 times in a row, he quickly corrected his course.
8. Why did the applicant leave? Chris's family moved to Washington state and it was impossible for him to participate.
9. Would you hire the applicant again? Hands down and absolutely, yes.
10. Would you have any hesitation or reservatiions about recommending the applicant to serve as our nanny or caregiver? Not at all. He's a great guy.
